What is Atrial Fibrillation?

Atrial fibrillation is a heart condition that causes an irregular and often abnormally fast heart rate. It increases the risk of a blood clot forming inside the heart. If the clot travels to the brain, it can lead to a stroke.

People with Atrial Fibrillation are 5 times more likely to suffer a stroke. Yet 66% of AFib strokes can be averted with treatment.

What causes Atrial Fibrillation?

With Atrial Fibrillation, the heart’s upper chambers will contract randomly and sometimes at high speed to the point where the heart muscle cannot relax properly between contractions.

Normally, the muscular walls will contract to force blood out and around the body, then relax so the heart can fill with blood again.

The cause of this is not fully known, but tends to affect people over the age of 65. It can be triggered by certain situations, such as drinking alcohol in excess, and smoking. There is no hard and fast rule however, as perfectly healthy people can succumb to Atrial Defibrillation.

    • Feeling faint

Treating Atrial Fibrillation

With appropriate treatment, the risk of a stroke can be substantially reduced.

it is possible to be treated by your GP,  and be given blood thinning medications to reduce the likelihood of blood clots. Anticoagulant medicines make the blood take longer to clot, making it a vital part of helping prevent strokes.

In more extreme cases, you may be referred to a Cardiologist, or more specifically, a cardiac Electrophysiologist. They will focus on your heart’s timing, electrical system, or diagnosing and treating irregular heartbeats or arrhythmias.

Prevention of Atrial Fibrillation

As is fairly standard, the main way to prevent Atrial Fibrillation is by maintaining a heart-healthy lifestyle. This includes exercising, avoiding excessive drinking, not smoking, and eating a healthy diet.

If you have concerns about your heart rate and blood pressure, visit your GP. They may recommend purchasing a blood pressure monitor to keep an eye on certain levels. Make an appointment with your GP if you notice a sudden change in your heartbeat or your heart rate is consistently lower than 60 or above 100.

Professional Blood Pressure Monitors

Some monitors are too advanced to be viable for home use but are perfect for doctors, nurses and other medically trained professionals.

The Omron HBP1100 and the Omron HBP1300 are the link between home and professional monitors, being suitable for both environments. Both offer amazing functionality and accurate measurements as well as being clinically certified. The HBP1300 even comes with a handle and shook proof bumper so is perfect for someone on the go.

Truly professional blood pressure monitors such as the Welch Allyn ProBP 2400 are not suitable for home use. This BP machine can detect irregular heart beats and uses microprocessor-controlled deflation rate which provides consistent technique. It also comes with a manual mode so it can be used much like a sphygmomanometer.

The most high end BP machines can reach up to £2000, and are rarely found outside of hospitals or private clinics. An example of such machine is the Spacelabs 90217. It utilises a proprietary oscillometric algorithm which has been proven in over 20 years use in paediatric, adolescent, adult and elderly patients with a variety of disorders. It also includes advanced Artefact Rejection Algorithms which discriminates between pressure signals, patient movement and respiratory artefacts. BP monitors like this should only be used by medical professionals.

Reduce Preventable Infections from Spirometry Testing

Vitalograph’s Bacterial Viral Filters Could Save Practices Thousands

Between early October to mid-January, figures from the Guardian say, 120 people died of flu-related symptoms in the UK, nearly three times as many as the previous year. With up to 30,000 people visiting a GP each week with influenze-like symptoms, the chance of infection spreading during spirometry tests is much higher.

Using one-way valve cardboard mouthpieces during spirometry testing provides very little protection from bacteria and viruses passing straight through the spirometer, potentially infecting the member of staff administering the test. This lack of protection can lead to cross infection between patients, lost work from staff illness and increased treatment costs later on from infected patients.

The Solution: Vitalograph’s Bacterial Viral Filters

According to Dr Derek Cramer and Simon Ward, of the Royal Brompton Hospital, using a BVF is common practice in lung function testing in secondary care, however, it is far less common in primary care. By assuming any patient taking a lung function test is infectious and using a BVF for all patients, GP surgeries could dramatically improve their rates of cross infection and protect their equipment, staff and patients.

How To Choose The Best Pulse Oximeter For Your Doctor’s Bag

Pulse oximetry is a simple, noninvasive and reliable method for rapidly assessing arterial oxygen saturation (SaO2/SpO2) and pulse rate in patients. It is a useful tool for assessing both adults and children.

The main indications of pulse oximetry include the assessment of acute respiratory infections, asthma, COPD and heart failure. In these situations it can provide valuable information about the severity of the illness and help when deciding on the need for hospital referral or admission.

What Are The Guidelines?

In the General Practice setting, the importance of measuring oxygen saturations has been emphasised in the NICE guidelines on COPD, the SIGN guidelines on asthma and community acquired pneumonia and the NICE sepsis guidelines. Assessing oxygen saturation should be used alongside clinical assessment, not as a standalone indicator.

Additionally, pulse oximetry helps to ensure that hypoxic patients are treated appropriately with oxygen. The NICE sepsis guidelines highlight the importance of assessing oxygen saturations when risk stratifying patients.

How Much Do They Cost?

All-in-one pulse oximeters can vary greatly in price from under £10 to over £200.

How Do I Choose A Pulse Oximeter?

There is so much choice available that it can be difficult to know where to begin. When selecting a pulse oximeter for your doctor’s’ bag, ensure that it is:

  • Lightweight & Portable – Keeping your doctor’s bag as light as possible will make your life much easier when you’re on the move.
  • Simple-To-Use – With just 10 minutes per consultation, the quicker you can perform an examination, the sooner you can help the patient. A simple-to-use pulse oximeter opens like a crocodile clip and should be able to be used on a wide variety of patients.
  • Cost-Effective – Nowadays, you don’t need a top-of-the-range model to see great results. A £30 model will serve as well, if not better than a £100+ model as they are often more lightweight and aren’t as expensive to replace if they become damaged or go missing.
  • Reliable & Accurate – It should go without saying that when assessing a patient, accurate and reliable results are essential. Look for models with signal strength indicators as one of the main causes of inaccuracy in oximetry is incorrect finger insertion.

Additional useful features include being easy to clean (by wiping with a 70% IPA swab, for example) and battery-saving features such as automatically turning off when a finger is withdrawn.

A Note On Assessing Children

When assessing children make sure you invest in a paediatric pulse oximeter as obtaining a correct fit is important to give an accurate reading. In the past, many GPs had to resort to using an adult pulse oximeter to try to assess a sick child. The readings that resulted were often incorrect, if they could be obtained at all.

As you can see above, the adult pulse oximeter (right) is too large to be used effectively on paediatric patients. The two paediatric pulse oximeters fit much more closely, keeping the sensor in contact with the skin and giving more reliable results.
